We work together to develop a personalized care plan specific to your patient’s needs.  Our Specialist will consult with you to help form the best care plan available to stabilize your patient and provide ongoing support.

 We also offer follow up consultation services for hospitalized patients that may need alterations to their plan as their care and recovery progress.
